Streams the monitor could not currently reach
This view includes offline, unreachable and upstream-error states. Restricted and rate-limited endpoints are intentionally not mixed into this failure list.

Why “unreachable” is not always permanently offline
Live media infrastructure changes quickly. A DNS lookup can fail for a few minutes, a CDN edge can return 5xx, a stream can rotate to another endpoint, or an upstream server can stop accepting requests from the monitoring location. This page therefore reports the latest observed state, not a permanent label.
Before removing a public directory source because an endpoint appears here, compare the latest result with its 24-hour and 7-day history. Repeated failures over many samples are stronger evidence than one timeout. Source playlists are also re-fetched separately, so an individual endpoint can fail while the M3U file that referenced it remains healthy.
If you are troubleshooting your own playback, compare this data with the M3U playlist troubleshooting guide and 403 restriction guide.
HTTP 401, 403 and 451 responses are stored as restrictions because the endpoint may work from an authorised geography or request context.
